Implemented By - The name of the person who pushes the Implemented button is entered here.  (Anyone who is an Implementer can do this.)

Date Implemented - After pushing the Implemented button, today's date will automatically be entered here.

Results - After verification is complete, the Verifier will enter a description of the results of the verification.

Passed or Failed - After verification is complete, the Verifier will push either the Passed button or the Failed button.  The Investigator is automatically notified by email with the results of the verification.

Verified By - The name of the person who pushes the Passed or Failed button is entered here.  (Anyone who is a Verifier can do this.)

Date Verified - After pushing the Passed or Failed button, today's date will automatically be entered here.
