Root Cause(s) - Enter all the root causes that you find for this CAPA.

Root Cause Codes - Enter all the Root Cause Codes that apply.  Note that while in this case there is one code for each root cause, you might have one code for multiple causes or multiple codes for one cause.  The codes are used for trending over time - a configurable pareto chart is available in the Reports section which shows counts of these codes.  If you have any supporting documents for the root causes listed, those files should be attached to this CAPA record and referred to in the Root Cause field.  A code can be deleted if needed.

Estimated Annual Savings - You may enter an approximate dollar amount that would represent an expected amount saved during the year following implementation of the solutions.

Solution - For each Root Cause, enter the solution that will permanently resolve the problem.

Verification Method - Enter the method to be used to verify the solution.

Implementer - Select the name of the person who will be responsible for implementing this solution.  Note that during Solution/Verification stage, the Implementer can reassign implementation to another person.

Verifier - Select the person who will verify this solution.  If practical, select the same person who originally reported the problem.  Note that during Solution/Verification stage, the Verifier can reassign verification to another person.

Verification Due Date - Enter the date where the Verifier will begin to be automatically notified that the solution should be verified.  Note: If your LOS can read data from your MRP/ERP system, then the verifier can be notified based on the next job run.  Note that during Solution/Verification stage either the Implementer or the Verifier can change the Verification Due Date.

Root Cause Completed - Push this button to complete the Root Cause stage.  The Root Cause field must be filled in and at least one Root Cause code is required.  All the required fields on the Solution & Verification page must also be filled in.  The CAPA Approver will be automatically notified by email. 

Completed By - After pushing Description Complete, the name of the person who pushed Description Complete will be entered here.

Date - After pushing Description Complete, today's date will automatically be entered here.
